I know somebody posted the form here on FT a couple of years ago. I'm sure it's been updated. The form is called a "Complaint/Compliment Form." The easiest thing to do would be for someone to ask for a form under the ruse of telling the screener you wanted to submit a compliment. I'm sure they would give you as many as you wanted. Then, it would be a matter of scanning it and posting it somewhere.
