I had to renew my South African passport a few years ago with valid Schengen and US visas, as well as UK residence permit still in them.

SA clipped the front and back pages, leaving the rest in tact - as they explained to me, the visas themselves remain the property of the issuing country, so they have no right to damage them.

My wife now travels with 2 passports (old and new), plus a certified copy of our marriage certificate. This gets around the name change issues.
