The ones I've seen have no options for PAX to select miles over voucher.

Purser writes up the issue on the form, (real brief note) dates and signs it with staff number and flught and pax name, and WHQ mails a $$$ voucher in due course.

Purser has a book with applicable amounts liable if you really care to ask,
