Perhaps I've been on different flights, but with the exception of an American Eagle flight, all of my recent AA flights (a combination of intra-California, mid-cons, transcons, and international) have pre-boarded passengers who need a bit more time/assistance to board, prior to commencing boarding for everyone else. On one or two flights, I think they boarded First at the same time.
Pre-boarders on my AA flights have usually been families with babies or very young children and people who have difficulty walking. The GA has never made an announcement about "medical conditions," but has usually done the "people who need a bit more time/assistance" PA. In general, they've identified those people prior to commencing boarding.

Last week I flew AA, ORD to MSP on an MD-80. 1 family with a baby tried to board with 1st and was specifically reject by the GA and told to board by their zone number.
