Spot on (and I've experienced all of these date changes - except the blue plus bit which didn't exist in my day...)
Basically the clock resets every time you go UP one tier - to the date that you earned that increase. Any surplus status miles when you go UP are also rolled over so you have fewer miles required to maintain or move up to the next level
If you stay at the same tier at year end, or if you slip down (i.e. you did not earn enough miles to move up a tier during the 12 months), then the starting date remains the same. And no rollover of surplus status miles either (although if you search the board carefully there may have been some, errr, "one time exceptions"
