I have not seen this behavior frequently but I did notice it on a friend's machine about a year ago. In her case, her browser was a bit old and an update seemd to settle things down quite nicely.

Of greater frequency, is behavior in which the UAL web site produced a list of moderately priced (i.e. discount coach) segments and then process the final itinerary as if it was full fare coach.
