The form isn't necessary. A quick note with a copy of your ticket receipt and the boarding pass from the alternative flight you took is all that is necessary. I've had four of these in the last month. UA 1k desk posted all of them within 48 hours of my fax. Regular MP desk takes a little longer; but its not a problem.
