Buy the Amex PREMIER Plus Cover - excellent package albeit expensive. It includes car hire cover in North America and Canada with means you don't have to buy CDW, LDW, PAI, etc. at ridiculous rates from the hire firm. I always hire a Hertz Prestige car which is actually a bargain once you take the insurances off.
Well worth the expense IMHO.
