I have had this happen two or three times in the past ten years at various places along with a couple of erroneous in-room movie charges.

I have heard of people asking for the mini-bar to be removed from the room, I wonder if most hotels will do this. I just don't like the hassle of trying to correct the bill and I am usually in a rush to get going at checkout time.
