I also ran into those issues with domestic upgrades not clearing multiple times. Reason: the ticket information must match the exact data of your upgrade request.
So - any schedule change that causes the flight number to chance essentially screws you up. Not only does the e-ticket have to be reissued, but the upgrade has to be re-requested as well.
