Yes, there were 2 of these in the kit and they were folded in with the survey, so I can see how you might have given them the heave-hoe, but you shouldn't have trouble replacing these. Call MP and explain the situation. Since these upgrades are "system traceable" they can void out the certificate numbers and issue new ones (if you go to the Mileage Summary on United's web site, you can see these listed on your account as awards issued on Jan 2, 1999). Once the old numbers have been voided, if anyone tries to use them, they will be kicked back to the ticketing/check-in agent as lost or stolen and be voided on the spot. So on these type of certs, United usually has no trouble re-issuing them if lost or stolen. Let us know how it goes!
