The same happened to me on a LHR-YVR flight last year in F. The CSD was quite insistent we fill out the forms. A week later, 15000 miles appeared in my & my wifes account. As it was first First, it didn't even bother me. I just enjoyed the fantastic service on the flight and read my book.
