The paperwork is the leverage - help the "patient", but don't fill in the paperwork - would be interesting to see if the AC "thankyou" changes. Over 10 years ago my physician wife came to the rescue on an AC YVR-YYZ flight and got 5K miles, seems that international medicine while not exactly lucrative stills pays better! BTW, my personal observation is that 50% of inflight medical emergencies seem to have an alcohol component.
