FWIW:

I've (unfortunately) had to cancel 3 int'l mileage award tickets (all on LX) but mine were all voluntary cancellations and so cost $100 a pop.

But I've always been refunded the taxes to the original form of payment (my AMEX). I cancelled the 3rd trip very recently and received the refund credit to my AMEX just last week.
