I didn't sign up for Bmi Diamond Club at all before I sent my letter and photocopies of my FB and AAdvantage statements. I sent it to the Diamond Club headquarters in Castle Donnington (address can be found on the Bmi website).

As I said in post #4, I received my new DC Gold card 7 days later.
