I joined KC under my work corporate membership (although work doesn't pay for it - we pay personally but get the discount rate).
The KC rep rang me about something in my application & I asked if I could make a change the card listed on the form to another & just give the details over the phone. She said that was fine, did it on the spot while I was on the line & the card turned up a week later. Maybe you could send the form in without your CC details, and write a note on it saying that you will call them (or ask them to call you) with the details?
Sounds like you've been stung badly before, I don't blame you for being cautious...
