On LH.com? Your number should have been added to all segments in the booking. It could be that LH and JK take longer to credit miles than UA?

If you had a change to the ticket on the missing flights that could explain it too. When the flights haven't been posted after 30 days you can send your boarding passes in to UA for credit. If your boarding passes are lost you can contact LH by email to get proof of your travel sent directly to UA for $20; not sure if JK offers this service as well.
