This is quite doable. You will have cleared French immigration before boarding the train in London so when the train arrives at Gare du Nord, you can just walk through the station to the street. I would go out to the front of the station and get a cab to Gare St. Lazare. It should cost 15 Euros or so and take roughly 20 minutes, even in afternoon traffic. There is really no reason to get to Gare St. Lazare early since (unlike the Chunnel) there is no requirement to be at the station early, and usually they don't allow boarding of trains until 10-15 minutes before departure anyway.

Obviously you should have your train tickets beforehand, with reserved seats, and you should have enough Euros for the cab too.

While some may note that you could do this on the Metro and you could, I wouldn't suggest it unless you are already familiar with the Paris Metro system.
