The EQM (1 per $) is a Chase issue, not a United issue, as it is Chase that handles crediting of the EQM. Chase's system handles that automatically as long as your purchases appear to be from united.com, meaning that the ticket is issued from the ROSEMONT, IL office. If a ticket issued by Web Support comes from the Rosemont office, you should be just fine... if it is issued from another office, you will not receive the EQM bonus, and I have no idea whether or not Chase Customer Service will then give you the bonus manually upon request. But, it's not something United will handle, as it is a Chase issue that you should take up with Chase customer service.
If you book a ticket through Web Support due to failure of united.com, you should still qualify for the 500 RDM "online booking" bonus, although it may not post automatically. If it doesn't post, call Mileage Plus 6 weeks after the flight, and if they say it wasn't an online booking, then explain it was booked through united.com's Web Support which is supposed to be the same as booking online (because Web Support only books tickets when there are issues with united.com, and they don't charge the normal $15 phone booking fee, so therefore it should, in principle, be the same as booking online). IME, they'll give you the RDM bonus. But contact Chase regarding the EQM, not United.

formite mentioned and as mentioned above, the booking has to be through united.com, which equates to having the charge made by the Rosemont, IL office. All other United charges will not earn the bonus EQM.

Is that true even for Web Support phone-issued tickets? I agree that tickets issued by calling Reservations would not be issued from Rosemont (though they may not be from Pune Maharas, either... it all depends on which call center you get!), but IIRC tickets issued by Web Support are issued through Rosemont just as if they were purchased from the website directly... can anyone confirm/refute this?
