That's a very important point. Even if you take the gas station recommended by the rental car companies (exit Somma Lombardo (?) directly located at the exit), you'll see that it's a full self-service and that they don't accept credit cards (at the moment?) there. So leaving MXP on the highway and using the AGIP station after 2-3 km would probably be the best option.
