1) Absolutely they do. I think it's scheduled for the year 2525. Zager and Evans wrote a song about that.

2) I've seen cases where the flight crew handed out compensation forms to the entire plane (award was a paltry 1K miles). I've seen other cases where they did nothing and you need to ask individually. I've seen one cases where they gave out free drinks; not sure if that was according to protocol. Unfortunately, I've probably had a half dozen cases where my own entertainment system was dysfunctional in transpac C or F; compensation ranged from 5 to 10K miles; last time I opted for a cash voucher; it was $200.
