I am quite sure the answer to #3 is no. The relevant part of the star file does not list PHX as a transcon city:

I believe since the aircraft stops, it counts as two segments, and is sold as such on the AA website. And thus, you should get 120 tier points (2 x 60). It is not on the transcon list.
