The last time I had trouble using an e-cert, I called web support, and they told me to book it online (without the cert) and call again to get the e-cert applied since the online engine wouldn't take it. I've heard some people keep web support on the phone WHILE booking, just to make sure all goes well. Of course, be aware that web support is handled by the ICC, FWIW.
