When I was gold equivalent on UAL, I got upgraded on my Pacific flights almost 50% of the time. I'd just get called up to get my new boarding passes at the gate. I don't know how it is nowadays. That was late 90's. Economy wasn't even packed. It really was great!

NW only did an upgrade once for me internationally in almost a million miles, just because it was a full flight.
