The only ones i can think of are:
News Orleans
Houston Hobby
Orange County
When things were starting to become very bleak for Midwest financially, they began cutting routes and frequencies, especially in Kansas City. The routes were:

-Austin
-Boston
-New Orleans
-Ft. Myers (which was operating seasonally)

In late 2002, Midwest had also eliminated service from Kansas City to Atlanta and Washington Dulles. Newark was supposed to be started in November 2002, but advance bookings were so weak that the flights were pulled before service even commenced.

Kansas City took a beating after 9/11 and it took a very long time for traffic at the airport to re-bound. Things are thriving there now and Midwest has been well received in the market place, thus all of the expansion.

Boston has since returned and has performed quite well. New Orleans also came back put was cancelled after Hurricane Katrina.

Other routes that Midwest operated in the past from Kansas City included Omaha, Des Moines, Raleigh-Durham and Orange County (dumped in favor of LAX).

With the regional jets coming on-line, I wouldn't be the least bit surprised to see Austin, Raleigh, and even New Orleans return. Also, Midwest is yanking the 717 of MCI-PIT starting May 1st.
