Further to my earlier reply, I notice that American Express anticipated and answered this question here...

Look at the paragraph titled "
The difference between the Premium Car Rental Protection (PCRP) and Car Rental Loss and Damage Insurance (CRLDI)" In particular note the final item "
CRLDI only covers vehicles rented in the United States, Puerto Rico, and the U.S. Virgin Islands. PCRP offers worldwide coverage, except for vehicles rented in Australia, Ireland, Israel, Italy, Jamaica, and New Zealand." This appears contrary to the understanding that the card already covers rentals in "most countries".
One aspect I do not like is that "Coverage is automatically extended to other American Express Cards". We have several American Express accounts and it might be appealing to enroll one card in this program and use that card to pay only if we wanted this benefit when (for example) renting in another country. It's easy enough to work around, but nonetheless irritating.
