Citi changed this for me around May 2005. My main problem with this system is that I typically run through expenses before I receive the statement (checking the transaction amount as soon as it posts to put on the expense report), and I often forget about the one line at the end of the statement cycle. I usually don't have a problem correcting this, but you never know how the expense nazis are going to react day-to-day (like when you get your expense rejected for taping receipts to A4 paper instead of letter paper).

SPG Amex notes the conversion rate is +2%, and is included with the transaction amount.

For personal charges, I always try to use cash as ATM rates are approx +1%, and I use ATM cards with no fees.
