Immediate family generally applies to spouse/children and parents and siblings as well - though as stated Lounge staff will usually want to check BPs (BTW we have to see them at LHR regardless so the staff weren't necessarily being suspicious-minded!) to make sure surnames are the same.
I'd be pretty surprised if access was refused to parents/siblings.
