The Bvlgari has a great spa (maybe the best in europe). Also the hotel service can be very good if they decide you deserve it (they are very snobbish though, you have to be good enough to stay there is their attitude -- hence the many reports on the web of poor service). Price is double the FS, but worth it. I'm never staying at the FS Milan again. The Bvlgari has blown both the FS and PH out of the water (at least at the high end).
I didn't find them snobbish at all, especially as they were on the young side and there is such a imaginable gap between them and most of the guests in terms of age and social/ financial standing. I think regardless of who the guest is, the prices at these Milan luxury hotels ensure that most people are indeed a certain kind, and so they are quite friendly and properly polite (although, I did observe that the young men standing at the front did look a bit coldly at some tourist types who stood at the entrance with cameras, gawking at some of the beautiful women having drinks in the garden). What I did notice, however, is that they were nice but they lacked the smoothness and confidence that comes with experience, as far as hotel personnel are concerned. Mind you, PHM is not really up there in terms of service either, but it's good enough.
In fact, when I asked to see a room, the people at the reception were very accomodating and actually took me on a full tour of the hotel, including the spa. Have to agree that the spa, particularly because of its newness, looks quite impressive, and this is probably among the best ones in a city hotel in this part of the world.
I guess it all depends on what each person is looking for because all three hotels are good but are very different.
