Mileage credit when there's a name mismatch

Here's the situation. My SkyMiles account lists me as Mr. MIDDLENAME LASTNAME, which I've always gone by in my life. Even flying overseas in the past, there's been absolutely no issue with getting tickets and, of course, subsequent mileage credit.

However, I've signed up for a trip with a group to AMS/CAI/TLV in February flying on NW and KL. The travel agency arranging all these details has insisted on my full name (FIRST MIDDLE LAST) as shown in my passport so I don't think there's anything I can do in getting my ticket listed as just "MIDDLENAME LASTNAME" to match my account. I will get my SM number printed on the ticket, but because of the name mismatch, I know the miles won't be automatically credited. So I'll have to resort to faxing in the BPs after the trip.

What is everyone's experience with DL in granting mileage and MQM credit when there's a name mismatch such as an extra name, initial, etc on the ticket? I'll need these approx 13K MQMs since I project that I'll actually earn elite status (albeit just silver) for the first time in my life in '07.
