Top restaurants for fish are Il Porto (in the Navigli area, be sure to make a reservation loooong in advance), Riccione and Il Grissino. They also do pasta.
Il Girarrosto and L'Assassino are excellent, rather conservative Tuscan restaurants.
Cracco Peck is tops, Gold (the new Dolce & Gabbana restaurant) is trendiest.
If you would like to try Milanese cooking, the best place is Antica Trattoria della Pesa. But you'll find few locals there, risotto is something we cook at home and rarely our choice when dining out.
