Well, both of these have already come to pass on AA, although it is limited.
Tickets between North America and Europe, India, Asia and Latin America booked in O and Transatlantic tickets booked in Q are not eligible for mileage credit.
Further, I fares and Q fares to certain international destinations are not upgradeable (unless one gets lucky
).
