Some transactions ran over the Visa network and incurred a fee, some ran over the ATM network and did not incur a fee. As you probably know, bank ATM's do not charge usage fees in most of Europe. So get a NON-Visa ATM card and you will not incur a fee at HSBC ATM's but HSBC US does add a fee to any transactions at a non-HSBC anywhere.
Note that you need to make sure your ATM card has a "Plus" or "Cirrus" logo on it. Most other ATM networks that are popular Stateside (NYCE, Star, Shazam, MAC, etc.) don't exist overseas.
