two segments, one flight number: upgrades deducted and promo miles?

Quick question that I couldn't find an answer to using search.

If you fly on a through flight (AAA-BBB-CCC with two segments having the same flight number) I know that you only receive miles as if it were a nonstop flight as the crow flies from AAA-CCC.

However, I am confused about two more complicated possibilities:

1) What happens if there is a double miles promotion from BBB-CCC? Do you qualify? You did actually fly the BBB-CCC segment...

2) Also, how many upgrades get used? If AAA-BBB would use 2 upgrades and BBB-CCC would use three, but AAA-CCC would only need four upgrades, are four upgrades deducted or is it five?
