2/200 times I have had it happen the planes were way oversold, I had a high coach fare code and hence the UGs.
Last time it happened was EWR-PEK...what a nice EUA that was...they also companion upgraded my plat boss. Thanks CO!
What you describe is referred to as an operational upgrade, or "op-up," as opposed to an EUA. More rare on CO than on some other carriers (UA comes to mind), but they do happen. I've received two in the past three years.
