Just enter the account holder's name as if he or she were the credit card holder. IME, the charge will go through anyway. Never had a problem with it with family members. I don't know if it's that they check only the last name, or don't actually check the name at all, but I suspect the latter - I think the values ordinarily submitted by the merchant for an authorization are card number, expiration date, and optionally CVV and AVS (house number and zip code).
