Another SICK calculation, based on one C-Class RTW:

5 stopovers allowed on each continent. 15 max. Make that 5 each in the USA, Europe and SE Asia. 5 days in one country/region should give you enough time to rack up around 40 segments (the short distance ones, ie around 100 miles) = total 120 segments. The US segments are counted as F-Class.

40 x 3750 miles = 150000 miles
80 x 2500 miles = 200000 miles
long distance RTW flights = around 60000 miles
rt flight to JNB in C = around 30000 miles

Total = 440000 miles

All done in less than three weeks.

The remaining 60k miles can easily be achieved in the remaining 49 weeks of the year.

Requirements: Find a nice ticket agent who will take three days off for ticketing this, plus a good radiologist and psychiatrist.
