The last point isn't that absurd. I was waitlisted for a BOS-SJC upgrade this past spring. The long ORD-SJC leg had one empty seat a couple of hours before departure, I wasn't first in line, it didn't look good, but the agent said I could jump ahead of all the other EXPs waiting for free upgrades if I was willing to burn a VIP or miles. I said "VIP" and the upgrade was confirmed on the spot. I never felt it was wasted.
