One pays the luxury tax, I believe about $37 US or 20 GBP when departing London on a business or first class cabin (ie upgrading with a VIP, using miles for upgrade). The luxury tax is only charged on departures out of LHR, LGW, etc but not flights going into those cities. Hope this helps.
