You can email Bangkok Air, they will make the reservations, send you a confirmation and you pay at the airport.
You can also buy airpass coupons (minimum of 3) for $50.00 each ($55.00 after Nov). You make reservations for the first leg-PNH-REP, the rest can be used on any flight with space available-domestic Cambodia, Thailand and Laos. You should check thoroughly about REP/BKK (if you are going that direction). Last fall, you could use the domestic coupons for REP/BKK. This spring, REP/BKK was reclassified as international. But in Sept, I know of two people who used the domestic coupons for REP/BKK.
