On a domestic fare, elites are entitled to mileage upgrade on any fare class, which is why nwa.com offered you an upgrade to F for 30K R/T. Non-elites are entitled to mileage upgrade anything higher than a T class fare, so they would have likely seen a pay $ to upgrade fare + mileage option, if offered.

On Trans-atlantic flights, only B class fares and higher are available for mileage upgrades, which is what you might be thinking of when stating that "fare rules usually prohibit upgrading anything lower than B class."

This thread just reminds me that I'm having the worst time finding mileage upgrades on a T class fare out of HNL.
