It was made clear to me and backed up by the T&Cs that whatever the degree of address verification, cardholder-not-present transactions were NOT guaranteed. If a cardholder disputed a transaction I got a chargeback with little or no investigation from Visa/mastercard. Signed delivery notes, proofs from couriers, faxed copies of cards etc meant little.
I was told (off the record) that statistical analysis was widely used. Ie only if a particular cardholder repeatedly disputed transactions would they be less biased in their favour.
I don't know if this is of any interest to you; my credit card was defrauded via an internet transaction at Office Depot in the US (I am UK based). The credit card company chargedback Office Depot and I saw the credit on my account. 3 months later Office Depot represented the credit card charge and it hit my account a second time. This time the credit card company ate the charge.
