The email I got in answer to my enquiry read:

You can earn miles on all Malaysia Airline System flights except Miri-Mulu, Zurich-Kular Lumpar, Tawau-Tarakan and also all internal flights within Malaysia and Australia.

This was in February 2003. At the time, only domestic MH flights were listed on the KLM website as non-qualifying.
However, the KLM website now reads:
<font face="Verdana, Arial, Helvetica, sans-serif" size="2">
You can earn and spend Miles on all scheduled flights operated by Malaysia Airlines, except between Tawau and Tarakan. You cannot earn Miles on any of the domestic flights in Malaysia or on any domestic flight in Australia
</font>
I do wonder if the specific inclusion of the Tawau-Tarakan flight means the ZRH-KUL is now qualifying.
