LH vs. LX vs. OS logo on M&M cards

I see that there are some M/M cards that don't have the LH logo in the top left hand cordner, but instead have the Swiss or Austrian logo.

What determines which logo goes on? Country of residence? For instance, for someone with a LH-branded card moving from UK to Switzerland, would he get an LX-branded card? Or is it determined by which airline you applied for your M&M membership? In this case, ex-Swiss Travelclub members would then have the Swiss logo, irrespective of their country of residence?

The other thing which is of course interesting: are there any differences in treatments, privileges, lounge acces, mileage programs, etc. between the different cards, depending on its "airline logo"?
