The first recorded occurance of it (that I've found) on FT is in this thread. The first reference to it is in this thread on US Aviation by our friend PineyBob (i.e. where BoeingBoy first heard it). ClueByFour later responds "You're Welcome" to Piney later in that thread.
Plaudits then to ClueByFour who shall forever get credit for coming up with the terminology that so aptly describes what many of us face on a daily/weekly/monthly basis!
