I do not know if it is targetted, but if you get the new card then the promo (100 miles per transaction, up to 15) goes with it. I don't know what gets you a new card, mine just showed up in the mail and it was not due to expire yet.
As far as 15 transactions of $1 each, as far as AMEX is concerned that is 15 transactions, thus a 1,500 mile bonus. The kicker is don't try doing it as both the gas station and AMEX will think it is possibly fraud or a system error and will lock/freeze your account. It didn't happen to me but I came close. A call to AMEX would get it unlocked pretty easy (with the proper security answers).
