This sounds like one of the Chase Cash Rewards Plus or Chase Cash Plus Rewards or whatever they're calling 'em these days.

There was a version of this card available for a while where the points could be exchanged for miles in a couple of FF programs (UA, CO and BA, I believe). But I don't think they offer that card any more. I think they only have the 5% cash back card, which is arguably a more valuable benefit anyway.
