End of story:

CSC never answered my letter. On Oct 25, I sent a copy to Vicki Escarra. Today, I received a call from one of her assistants.

He would not budge on the MQM's or the GM bonus, but did agree that I should have been given full mileage on the El Al segments. He also agreed to give me the extra 5,000 miles that Amex had promised but not delivered. (They had sent me a letter offering 15,000 to sign up for Gold card, but I received only 10,000.) Together, this gave me 6747 miles.

Having burnt up 105,000 miles recently (40,000 for upgrades, 25,000 for a domestic r/t ticket and 40,000 for a TLV-MXP r/t, I was getting very low. This 6747 put me over 40,000 again meaning that I can get another European trip in.)

I am not thrilled about the MQM's but got some satisfaction in his promise that he would send a memo to all city offices telling them to notify customers when booking them on flights where MQM's are not being given and letting them know of available alternatives.
