You may have to ask for the upgrade at the gate. I flew NAS-IAH in 2002 and did not get an EUA. When i asked at the airport the staff had no idea what I was talking about but upgraded mrs. iwantanupgrade and myself just to get me to shut up. IIRC CO had just started this non-stop flight.
