For me, the sticking point is still 100% MQMs on cheap fares. Without that, my company's policy of buying the lowest fare is likely to prevent me from being able to qualify for PM. And if I don't have a shot at PM, I'll just shoot for CP instead.
Sometimes I'm in F/A/J, sometimes I'm in K/Q, but even inside of two weeks' planning I end up in insULT and am not going to pony up the difference between insULT and K+ just for more MQMs, especially with the $100 change fee for buying up on my own nickel, often for $200+.
